IMPORTANT NOTICE OF AVAILABILITY OF TAX FORM 1095-B and 1095-C

In compliance with the Affordable Care Act (ACA), as amended by the Paperwork Burden Reduction Act (PBRA), Saint Joseph’s Center is required to provide Form 1095-C to covered individuals (upon request) and to the Internal Revenue Service (IRS).

Form 1095-C is available electronically through UKG for current active employees. Individuals who are no longer working at Saint Joseph’s Center may use the information below.
